Declaration of Death

  1. Death Inspection Certificate
  2. Family booklet of the deceased or their birth certificate.
  3. The husband’s or the wife’s birth certificate + a copy of the marriage certificate in case one of the spouses is deceased.
  4. The deceased’s National Identity Card (CNIE)
  5. Note: the acknowledgment shall be submitted within a maximum of 30 days
