  • Birth Certificate
  • Identification Record/Civil Status booklet or the Family Record
  • A Certified copy of the Marriage Act for Moroccans
  • A Full Copy or Extract of the parents
  • Acknowledgement of Paternity
  • Identity card of the declarant
  • Note: the acknowledgment shall be submitted within a maximum of 30 days

  • The Civil Status Office where the declaration of birth took place

  • The Civil Status Office where the declaration of birth took place
